Job Summary :

We are seeking a compassionate and dedicated Hospice Registered Nurse (RN) to join our team. This will be a direct hire, full time role. The Hospice RN will provide high-quality, patient-centered care to patients with terminal illnesses and their families in a hospice setting. This role is crucial in managing symptoms, providing emotional support, and ensuring the comfort of patients during their end-of-life journey.

Key Responsibilities :

• Patient Care : Deliver holistic nursing care to patients, focusing on comfort, dignity, and quality of life.
• Assessment : Conduct comprehensive assessments of patients' physical, emotional, and psychosocial needs.
• Care Planning : Develop, implement, and revise individualized care plans in collaboration with the interdisciplinary team and family members.
• Symptom Management : Administer medications and treatments as prescribed, manage pain, and address other symptoms effectively.
• Emotional Support : Provide emotional and spiritual support to patients and their families, assisting them in coping with end-of-life issues and decisions.
• Education : Educate patients and families about the disease process, symptom management, and available resources.
• Coordination : Coordinate care with the interdisciplinary team, including physicians, social workers, and chaplains, to ensure comprehensive support.
• Documentation : Maintain accurate and timely documentation of patient assessments, interventions, and outcomes in accordance with hospice policies and regulations.
• On-call Responsibilities : Participate in on-call rotations as required to provide support during after-hours.
