Shift : MondayFriday days with weekend rotation and on-call as needed

Job Description :

Sunbelt Staffing is hiring a Home Health RN for a travel contract in Andalusia, AL. The nurse will provide skilled nursing care in patient homes, complete OASIS assessments, and chart with Homecare Homebase (HCHB). This position offers the opportunity to work independently while providing compassionate care in the community.

Key Responsibilities :

Perform skilled nursing visits and assessments

Complete OASIS documentation accurately and on time

Manage patient care plans using Homecare Homebase (HCHB)

Provide education to patients and caregivers on health conditions and treatments

Communicate with the interdisciplinary care team to coordinate care

Qualifications :

Active Alabama RN license

Minimum 2 years of nursing experience, with at least 1 year in home health

Proficiency with OASIS and Homecare Homebase (HCHB) required

Valid drivers license, auto insurance, and reliable transportation

Ability to pass background check, drug screen, and onboarding in compliance with the Joint Commission
