Optimal Care is hiring a Hospice RN Clinical Manager in Bingham Farms!
This position has a $10,000 Sign On Bonus!
Key Responsibilities
Optimal Care Hospice Clinical Managers are Registered Nurses who manage the coordination, supervision, and implementation of professional and supportive services to clients in a safe, cost-effective manner. The Clinical Manager may assist in education and training related to clinical practice, regulation, and reimbursement changes.
In this role you will be responsible for :
Planning, directing, and participating in the delivery of professional services to hospice patients and familiesCommunicating significant findings, problems, and changes in condition or environment to the physician and other personnel involved with patient careInitiating a plan for patient safety, using patient, family, and community resourcesIdentifying deviations from normal nursing and normal patient condition statusDirecting all aspects of operations and care delivery in accordance with the mission and values of Optimal Care HospiceSupervising the coordination of services with providers, vendors, or facilities in the delivery of care and related issuesProcessing and coordinating patient referrals, physician orders, staff assignments, and patient re-certifications, and communicating between the referring physician, various caregivers, the patient and their familySupervising and providing clinical direction to the RN, LPN, Chaplain, MSW, Aide, Music Therapist, Massage Therapist, Volunteers, and Bereavement Coordinator to ensure quality and continuity of services providedMeeting with all field teams regularly to provide guidance and informationMaintaining adequate staffing levels to meet patient needsCoordinating services to ensure the availability and competency of staff to respond to agency referrals and provide assessment visits within 24 hours of referralCoordinating weekend on-call services for the field teamRequired Qualifications
Graduate of an accredited Diploma, Associate, or Baccalaureate School of NursingCurrent State License as a Registered Nurse2-3 years experience as a Registered NurseNursing skills as defined by accepted nursing standardsCurrent and valid driver's licenseReliable transportation and valid automobile insurance coverageKnowledge of business management, governmental regulations, and CHAP standardsDemonstrates ability to assess and respond to the needs of patients, families, and caregivers in varied settings and to function effectively in the home or nursing facility environmentDemonstrates skill in nursing practice, ability to cope with ill, disabled, terminal patients, family emotional stress, and tolerance of individual lifestylesDemonstrates ability to handle multiple priorities, documentation requirements, and deadlinesDesired Qualifications
BSN preferredHome Health or Hospice experience is preferredLocation

$85,000-$105,000Hours
