Schedule : 1.0 FTE Monday-Friday-between 6 : 30am and 5 : 00pm - Travels to assigned nursing home facilities, covering the Fox Valley from Appleton to Oshkosh (includes Kiel and Chilton) .

Call Schedule : 1 : 6

Practice Detail : Family Medicine practice in a nursing home environment. Provides onsite care to nursing home patients at various locations; works with a support team onsite to assist with patient care needs. Experience preferred. Visit 12+ patients per day in the skilled nursing facility. Responsible for schedule to assigned sites, or as needed.

Responsibilities

Ascension-Wisconsin is looking for a Board Eligible / Board Certified advanced Practice Provider to join our Nursing Home Team. This is a great opportunity for the right candidate to partner with Ascension-Wisconsin in the Fox Valley to provide medical services to patients.

Position Highlights :

Responsibilities :

In collaboration with a physician, provide for the expansion of individualized primary healthcare services by diagnosing and treating certain acute or chronic health conditions.

Obtain patient history and perform assessment via observation, interview and examination. Order, perform and interpret diagnostic studies. Perform preventative health assessments, screening, immunizations and patient care.

Provide direct treatment and management of health conditions via referral to other healthcare providers or community resources.

Counsel and educate patients and their families / caregivers concerning preventative health, treatment options and community resources.

Document delivery of health care and nursing processes in accordance with specific specialty area standards and network nursing policy.

Requirements

Licensure / Certification / Registration :

BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required.

Nurse Practitioner credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date required.

Registered Nurse credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date required.

Drug Enforcement Admin credentialed from the Drug Enforcement Administration preferred. Verified by local Credentialing Office.
