Job description

We have an exciting and immediate opportunity for a dedicated Family or Internal Medicine Physician to join a respected Primary Care group. This role offers numerous benefits and incentives, making it an attractive position for talented professionals.

Position Functions :

As a Physician, you will provide comprehensive care including diagnosis, treatment, and care coordination. Your responsibilities will also include maintaining electronic health records (EHR) while adhering to quality management and safety standards.

Duties and Responsibilities :

  • Obtain health history and conduct physical examinations.
  • Diagnose, treat, counsel, and coordinate care for patients.
  • Review and update problem lists and care plans at each visit.
  • Meet documentation standards, completing all progress notes within 24 hours post-visit.
  • Adhere to on-call responsibilities as per the rotating schedule, responding within 20 minutes.
  • Prescribe medications according to Illinois statutes and professional guidelines.
  • Ensure all activities align with our group’s quality standards and clinical guidelines.
  • Comply with Meaningful Use standards by utilizing certified EHR technology to enhance quality and efficiency in care.
  • Manage EHR In-Basket, delegating appropriately and responding to messages within 72 hours.
  • Involve patients’ families in care discussions and ensure their needs are met in accordance with clinic policies.
  • Encourage open communication for questions and concerns from patients and families.
  • Support patients in self-management of their care.
  • Conduct educational activities for patients regarding their health conditions and treatment plans.
  • Monitor the cost-effective use of clinical resources.
  • Collaborate with care teams and case management to ensure coordinated delivery of services.
  • Ensure appropriate transfers for patients presenting with higher acuity than clinically warranted.
