DESCRIPTION OF WORK

In this vital position, you will serve as a Social Worker within the hospital's Continued Care Service, assuming responsibility for delivering social service casework and actively engaging in community support planning processes that facilitate patient recovery and transition from the hospital. This role requires a working knowledge of social service and hospital policies, Centers for Medicare and Medicaid Services regulations, and current mental health legislation. It also involves knowledge of available resources to support patients with daily coping skills and effective discharge planning. Additionally, you will identify existing and potential challenges and contribute to developing solutions both individually and as part of a team.

Work Schedule and Additional Information :

• Full-time employment, 37.5 hours per week
• Work hours are 8 : 30 AM to 4 : 30 PM, Monday - Friday, with a 30-minute lunch.
• May change based on operational needs.
• Overtime and travel : As needed
• Telework : You will not have the option to telework in this position.
• Salary : In some cases, the starting salary may be non-negotiable.
