Job Description

The Social Services Practitioner provides clinical psychosocial services to patients and families across all age groups (neonate through geriatric). Responsibilities include :

  • Assessing psychosocial needs
  • Coordinating referrals to appropriate facilities or community agencies
  • Providing crisis intervention
  • Delivering bereavement counseling
  • Supporting patients and families in complex emotional or social situations

    • Requirements

  • Minimum 1 year of experience
  • BLS – Required
  • CPI or TEAM – Required

    • CPI may be completed onsite after hire if needed

    Education & Experience

  • Bachelor's Degree or equivalent – Required
  • Master's Degree in Social Work (MSW) or equivalent combination of education / experience – Required
  • 3 years' experience in social agency, hospital, or health / welfare organization – Preferred

    • Licenses / Certifications

  • Licensed Clinical Social Worker (LCSW) Preferred
  • CPI or TEAM certification – Required
