Job Description :

• Deliver comprehensive physical therapy services to patients in a home health setting.
• Develop personalized care plans to address patients' needs and goals.
• Conduct assessments and re-evaluations to monitor progress.
• Educate patients and their families on treatment plans and home exercises.
• Collaborate with interdisciplinary teams to ensure coordinated patient care.
• Document patient progress and treatment outcomes accurately and timely.
• Travel within the designated territory to provide in-home therapy services.
• Maintain compliance with all healthcare regulations and standards.
