Key Responsibilities :

• Coordinate with community providers to facilitate comprehensive care and referrals for families.
• Participate in collaborative case planning and case review meetings as required.
• Deliver essential case management services, including :
• Supervised visitation
• Parent aide support
• Behavioral aide assistance
• Transportation services
• Crisis intervention
• Advocacy and linkage to services, including drug screening facilitation if applicable
• Conduct in-home visits to monitor progress and provide ongoing support for families engaged with DFCS.
• Provide parenting education and model positive parenting strategies.
• Collaborate with external professionals (e.g., schools, therapists, case managers) to ensure service alignment.
• Maintain accurate documentation of services provided, including case notes and service logs, in compliance with contractual requirements and applicable standards.
• Provide transportation services related to family service plans (e.g., for visitation and appointments) as appropriate.
