Key Responsibilities :
- Coordinate with community providers to facilitate comprehensive care and referrals for families.
- Participate in collaborative case planning and case review meetings as required.
- Deliver essential case management services, including :
- Supervised visitation
- Parent aide support
- Behavioral aide assistance
- Transportation services
- Crisis intervention
- Advocacy and linkage to services, including drug screening facilitation if applicable
- Conduct in-home visits to monitor progress and provide ongoing support for families engaged with DFCS.
- Provide parenting education and model positive parenting strategies.
- Collaborate with external professionals (e.g., schools, therapists, case managers) to ensure service alignment.
- Maintain accurate documentation of services provided, including case notes and service logs, in compliance with contractual requirements and applicable standards.
- Provide transportation services related to family service plans (e.g., for visitation and appointments) as appropriate.
Qualifications :
Bachelor's degree in a human services-related field (e.g., Social Work, Psychology, Counseling, Criminal Justice) or a High School Diploma / GED with at least 5 years of relevant experience.Strong knowledge of family systems, trauma-informed care, and community resources.Reliable transportation, a valid driver's license, and proof of insurance.Willingness to complete required background checks and onboarding processes.Deliverables :
