About The Position
- Provide skilled nursing care to patients in their homes, ensuring continuity and quality of care.
- Assess, plan, and implement individualized care plans for each patient.
- Educate patients and families on treatment plans and preventive health measures.
- Coordinate with interdisciplinary teams to ensure optimal outcomes and patient safety.

Job Requirements
✅ 6 months–1 year of recent Home Health experience required
✅ BCLS certification required
✅ Must obtain MOAB Certification (4-hour de-escalation course provided first week)
✅ COVID vaccination required (medical / religious exemptions accepted)
✅ Must pass CYFD fingerprinting upon arrival (home visits may involve children)
