Position Description

A member of the Children's Long Term Support Care Management Team (CLTS) responsible for coordinating case management services provided to a group of members. The CLTS Case Manager locates, manages, coordinates, and monitors all proposed waiver services, other services, and informal community supports needed by eligible persons and their families.

Skills and Qualifications

• Have a four-year bachelor's degree in a social services area (e.g., social work, psychology, nursing, rehabilitation, behavioral disabilities, or related field).
• Have at least two (2) years of experience working with children with Autism.
• Must be knowledgeable of Wisconsin's long-term support funding sources and Medicaid Waiver programs.
• Must have knowledge of community resources to meet clients' needs and an understanding of long-term care options available in the community.
• Must possess a valid Wisconsin driver's license, reliable transportation, and car insurance.
