Since 2013, our specialized team has been serving vulnerable individuals who are unableeither temporarily or long-termto access care in traditional practice settings.

No weekends or holidaysMinimal on-callEach of the NPs has a caseload of assigned primary care patients within a designated geographic area within our overall service area. Our program serves portions of Albany, Rensselaer, Schenectady and southern Saratoga counties.The number of patients assigned to each providers caseload is significantly smaller than a typical primary care practice (between 150-185), with approximately five scheduled visits a day, allowing time for travel and longer visits due to the complex needs of our patient population.NPs start and end their day from home.Our Home-Based Primary Care Team is supported by :

REQUIREMENTS :
Applicants should have the following :
Board Certified- Adult Gerontology or Family Nurse Practitioner Certification / Internal MedicineNYS License and DEA certificationAt least 2-3 years of Nurse Practitioner experience working with an elderly population is required.Experience working as a Nurse Practitioner in a Primary Care setting is preferred.This is an exciting opportunity for the right Nurse Practitioner to join St. Peter's Health Partners!
