Salary : $50.00 per hour

This position will serve Humboldt Park, River Grove, Irving Park and the surrounding areas

Schedule : 1 day per week

$125 per completed visit

Key Responsibilities :

Conduct in-home visits, health assessments, and care planning for members with acute and chronic conditions

Collaborate with other healthcare providers to deliver high-quality, patient-centered care

Participate in call rotation of telephonic on call coverage after hours

Requirements :

Bachelors in nursing with Illinois APRN license (Masters preferred).

Board certification for Nurse Practitioners and DEA licensure.

Valid drivers license and proof of insurance.
