Gateway Rehab Center (GRC) has an opening for an LPN or RN for our extended care program in Waynesburg, PA. In this role, you will provide assessment, nursing diagnosis / treatment planning, and direct nursing care to GRC patients. This position receives supervision and instruction from the Executive Regional Director.

Responsibilities

• Completes new patient admissions.
• Provides direct patient care including medication administration.
• Documents patient assessments and nursing care rendered.
• Intervenes in crises and other emergency situations.
• Completion of audits as required.
• Interacts with the physician through coordination and the provision of assistance via in person, telephone, med wand or other platforms.
• Complete medical triage, document and report findings to medical staff.
• Training of PCT’s / Tech 4s to assist with self-administration.
• Sign off patients to self-administer medications.
• Collaborate with pharmacy on copays / refills and relay information to patients.
• Coordinates with all GRC departments as appropriate.
• Attends GRC mandatory in-services and trainings.
