Salary : $95,000 - $105,000 per year (depending on experience)

Position Overview :

We are seeking a highly motivated and experienced Registered Nurse to join our team as a Patient Care Manager in the Home Health sector, based in Roswell. This role is key in overseeing and ensuring the delivery of high-quality, compassionate care to patients within their homes.

Qualifications :

  • Education : BSN degree preferred or equivalent clinical experience.
  • Licensure : Current Registered Nurse (RN) license in the state of Georgia.
  • Experience :
  • Experience in home health care or a related field.

  • Previous experience in a home care setting with two years of management or supervisory experience is preferred.
  • Technical Skills : Proficiency in electronic medical records and healthcare technologies; knowledge of home health regulations and standards.
  • Soft Skills : Excellent communication and interpersonal abilities; strong organizational and problem-solving skills; leadership and team management capabilities.
  • Key Responsibilities :

  • Lead and supervise a dedicated team of healthcare professionals to deliver exceptional patient care.
  • Coordinate patient care services and develop individualized care plans.
  • Conduct regular patient assessments and monitor the effectiveness of care plans.
  • Ensure compliance with all healthcare regulations and standards.
  • Collaborate with multidisciplinary teams to optimize patient outcomes.
  • Provide education and support to patients and their families regarding health conditions and care plans.
  • Maintain accurate and timely documentation of patient care and progress.
