The PT is ultimately responsible for implementing and documentation of interventions, outcomes, discharge plans and patient / family / care giver education. He / she collaborates with physicians and other members of the interdisciplinary health care team to provide continuity of care. The Physical Therapist is responsible for the supervision of and delegation of tasks to appropriate individuals including Physical Therapist Assistants (PTA), technicians, students, and other customers of the organizations. The PT must demonstrate the knowledge and skills necessary to provide age-appropriate patient care and is accountable to the policies and procedures of the organization. He / she reports to the Director of Rehabilitation Services. Education : Graduate of a professional physical therapist education program accredited by the Commission of Accreditation in Physical Therapy Education (CAPTE).
