Occupational Therapist

Job Description:

We are looking for a dedicated and compassionate Occupational Therapist to join our home health team. In this role, you will be responsible for evaluating patients, developing personalized therapy plans, and delivering skilled interventions to support patient recovery and independence.

Key Responsibilities:

• Conduct occupational therapy evaluations and diagnostic tests under a physician-approved plan of care
• Develop, review, and adjust individualized therapy care plans
• Educate patients, families, and caregivers on treatment procedures
• Coordinate timely delivery of care and collaborate with interdisciplinary teams
• Maintain accurate and timely documentation
• Promote safety standards and adhere to infection control practices
• Participate in patient care planning and quality improvement initiatives

Qualifications:

• Valid Occupational Therapist license (OTR/L)
• Experience in home health or clinical settings preferred
• Strong communication, organizational, and time management skills
• Ability to work independently and as part of a team
