Position Summary :

The Home Health RN provides excellent patient care by assisting in the collaboration, development, implementation, revision, and reporting of the case management program. The RN acts as a liaison between the patient, family, nurse, physicians, multidisciplinary team, and the patient’s healthcare benefactor to optimize outcomes, while serving as a consultant to the healthcare team on specific patient care items.

Responsibilities :

Lead the coordination of patient care with other disciplines, ensuring appropriateness and timeliness of care.

Ensure the interdisciplinary care plan aligns with the patient’s clinical course, ongoing care needs, and covered services.

Monitor diagnostic testing, treatments, procedures, and other care aspects for appropriateness in acute care.

Collaborate with physicians regarding resource utilization, treatment planning, discharge planning, and estimated length of stay.

Manage complex cases including frequent ED utilizers, chronic pain / substance abuse patients, and homeless populations.

Coordinate transfers to tertiary centers, ensuring all necessary information for continuity of care is provided.

Provide oversight and collect regulatory / accreditation compliance data.

Conduct focused reviews of frequent ED visitors’ admissions and histories.

Submission Requirements :

2+ years nursing experience – Required

6 months of Home Health experience – Required

Travel experience – Required

Hawaii RN license – Required

Homecare Home Base & OASIS – Required

COVID & Flu vaccines – Required

Bachelor’s degree – Preferred

Case Manager experience – Preferred
