Nurse Practitioner Skilled Nursing Facility

The Geisinger Skilled Nursing Program is seeking an experienced Nurse Practitioner to rotate between Mountain Top Rehabilitation & Healthcare Center, Mountain Top, Birchwood Rehabilitation and Healthcare Center and Lakewood Rehabilitation & Healthcare Center, Nanticoke, PA.

Job Duties

This position is as a Geisinger provider in a contracted skilled nursing facility and requires an experienced advanced practitioner, prior experience working in a skilled nursing facility, family medicine, internal medicine or ICU. The advanced practitioner (AP) provides direct (hands-on) care in skilled nursing facilities with a goal to improve quality and prevent unnecessary re-admissions to the acute care setting.

Master's Degree-Physician Assistant (Required)
