Job Summary :

Under minimal supervision and at the direction of the Case Management Director, the Inpatient Social Worker / Patient Counselor helps patients address bio-psychosocial needs and navigate medical care and community resources. This role provides crisis intervention, short-term counseling, and supports patient transitions along the continuum of care. The Social Worker / Patient Counselor also coordinates discharge planning in collaboration with patients, families, and the healthcare team.

Required Qualifications :

• Master's Degree in Social Work or Counseling Psychology.
• One (1) year of experience as a Social Worker or Counselor.
