Hoffman Homes is a community of employees gathered on one campus to help youth and young adults understand and manage their mental health diagnosis. We are looking to add an LPN to our outstanding medical team (Monday - Friday schedule) with paid holidays off. As an LPN, you will be an integral part of our medical services team. You will be working with several RNs and physicians, in a collaborative effort, to address the medical needs of our youth / young adults of HH.

Shift : 7 : 00 AM - 3 : 00 PM

Duties Include

• Address any illness or injury concerns of the youth / young adults using basic nursing assessments and practices
• Prepare documents and files for admissions to the program.
• Develop and revise a healthcare report for each youth / young adult in care.
• Assist physicians by relaying medical information and directives to others.
• Transmit medication orders to pharmacy.
• Communicate changes in medical information between the doctors and the other treatment team members.
• Assist with the management of each patient file to be sure that all necessary information is in place.
