This opportunity is with a mission-driven healthcare organization providing proactive preventive primary care services to geriatric patients in skilled nursing facilities (SNFs) assisted living facilities (ALFs) and private residences.

Salary and Benefits

• $100000 base salary annually for meeting standard patient expectations
• Lucrative bonus structure :

Approximately $7000 extra for adding just one additional patient per day

Responsibilities

9 visits per day for home visits
