MDS Coordinator RN / LPN Responsibilities :

Coordinate the collection and submission of MDS assessments for our patients, ensuring timely and accurate completion

Collaborate with interdisciplinary teams to develop and implement individualized care plans

Conduct initial and comprehensive assessments, as well as ongoing assessments, to evaluate patient care needs

Develop and maintain accurate and up-to-date patient records

Communicate effectively with patients, families, and healthcare providers to ensure seamless care transitions

Identify and report changes in patient condition to the healthcare team

Participate in quality improvement initiatives to improve patient outcomes and reduce readmissions

MDS Coordinator RN / LPN Requirements :

Current RN or LPN license in the state of Iowa

Strong communication and interpersonal skills

Ability to work in a fast-paced environment with frequent interruptions

Proficiency in electronic medical records (EMRs) and MDS software

Strong attention to detail and organizational skills
