Work Shift : Day (United States of America)

Compensation Range : The base pay scale for this position is $23.35 - $35.51. In addition, this position will be eligible for additional benefits consistent with the role. The salary of the finalist selected for this role will be determined based on various factors, including but not limited to : scope of role, level of experience, education, accomplishments, internal equity, budget, and subject to Fair Market Value evaluation. The hiring range listed is a good faith determination of potential compensation at the time of this job advertisement and may be modified in the future.

What You Will Be Doing

Serve as the consistent clinical resource for questions and concerns from patients and staff. Perform scheduling and registration for patient appointments. Perform check-in and check-out for patient visits to ensure consistency when recording intake measures, including but not limited to blood pressure, weight, labs, and patient-reported outcome measures (PROMs). Receive incoming calls and perform outbound calls, serving as a liaison to our providers. Manage patient and collaborating provider requests in the EPIC. Under the direction of the physician or advanced practice provider, encourage adherence to plan of care. Collaborate with the care team to facilitate the patient journey and achieve desired outcomes and patient goals. Develop relationships with various teams within HSS, including physician practices, physical therapy, nutrition, exercise physiology, social work, weight management, and radiology. Work closely with the care navigation team to facilitate patient needs. Participate in and contribute to team meetings. Manage clinical information from patients and providers including imaging, laboratory, and medical records to ensure inclusion in the patient's HSS medical record. Answer triage questions as needed for the pre-access scheduling team. Provide insight into the care management process and work with administration to ensure patients can receive the care they need in a timely manner. Maintain clear and effective communication with all applicable program stakeholders.
