Supervisor Responsibilities : No
Location : 121 W. 154th St in Harvey, IL
Purpose : The Activity Aide implements recreational and activity services in compliance with CHC policies and procedures.
Responsibilities :
- Implements activity and after school programs according to the written schedule
- Document progress and attendance for all activities (both group and individual)
- Completes documentation daily
- Assists with completion and implementation of documentation of habilitation plans
- Maintains records and prepares requested reports
- Complete activity notes by the 10th of each month
- Participates in the continuing interdisciplinary evaluation of individual residents needs
- Assures there is written permission given by the resident's physician to participate in the activity program
- Implements the activities to coordinate with other departments to ensure communication between all departments to provide uniformity of goals
- Participates in transporting students to and from the activities
- Assist in the development and implementation of special events
- Assist in the coordination of holiday visits from various groups / foundations
- Suggest ideas for in-house and community activities
- Attends seminars / workshops / facility / department meetings as necessary
Qualifications :
High School diploma or equivalentExperience working with students with complex medical needs and intellectual disabilities.Ability to pass criminal background check and drug testingExperience working with various socioeconomic populations is a plus.Must be able to lift, push, pull or carry a minimum of 50 lbs.Benefits : Medical, Dental, Vision, Life, 401K, Tuition Reimbursement, Short- & Long-Term Disability, and other benefit options
