Case Manager

The PATH Case Manager plays a crucial role in supporting individuals who are homeless or at imminent risk of homelessness and experiencing serious mental illness. This position is responsible for providing comprehensive case management services, linking consumers to vital community resources, and ensuring a successful transition toward stability and independence. The PATH Case Manager reports to the Program Manager.

Responsibilities :

• Provide comprehensive case management for assigned consumers, ensuring their needs are met through a person-centered approach.
• Conduct outreach to individuals who are homeless or at risk of homelessness, establishing trust and building positive rapport.
• Assist consumers in accessing and utilizing community resources, including housing, healthcare, and social services.
• Assess consumer needs, develop individualized service plans, and link them to appropriate supports.
• Complete all required documentation in consumer files and in the New Jersey Homeless Management Information System (HMIS) in compliance with policies and regulations.
• Maintain up-to-date knowledge of each assigned consumer's situation, including key contacts and support systems.
• Collaborate with consumers to develop crisis plans that incorporate available 24-hour community services.
• Educate consumers about mental health, substance use, and crisis management strategies.
