Job Description

Job Title : Registered Nurse

Job Specialty : Home Health

Shift : Day 5x8-Hour (08 : 00 - 17 : 00)

Guaranteed Hours : None

Experience : Minimum 1 full year of Adult Home Health experience or 1 full year of Hospice experience

License : Must have an active RN license (no pending licenses accepted)

Certifications : CPR, BLS

Must-Have : Proficiency with HomeCare HomeBase (HCHB) and Oasis within the last 6 months

Job Description :

• Provide, coordinate, and direct the provision of home nursing care according to physicians' orders and agency policies and procedures.
• Responsible to the Clinical Manager.
• Directly supervise and evaluate Home Health Aides and Licensed Practical Nurses / Licensed Vocational Nurses.
• Conduct home visits and attend periodic office meetings.
• Utilize reliable transportation with a current driver's license and agency-required liability insurance.
• Average of 5-8 patients per day with productivity expectations of 30 points per week.
• Handle various types of cases, including Start of Care, Wound Care, Follow-up Visits, etc.
• Participate in on-call rotations as required, typically one weekday a week and one week a month.
• Perform charting with proficiency in HomeCare HomeBase and Oasis.
• Must be willing to travel up to a 75-mile radius from the home clinic.
• Mileage is reimbursed at the IRS standard rate.
• Local candidates accepted at the same rate.
• Completion of documentation within 24 hours of the patient appointment is mandatory.
• Be open to floating to nearby clinics if patient volumes are insufficient.
