LPN / Licensed Practical Nurse POSITION SUMMARY :

A LPN / Licensed Practical Nurse is one of the main support providers to our residents. The position aids residents in the Assisted Living and / or Memory Care Program(s) through assessments and care planning, medication and treatment management, implementation, and evaluation of resident plans of care, documentation of nursing services and enabling residents to lead a quality life while maintaining their dignity. The position will enhance the lives of older adults by assisting them with patience and a friendly smile all while embracing teamwork and collaboration among all staff.

Essential Duties :

• On-site visits at least every 45 days for completion of individualized assessments.
• Be available on call as required as per regulatory standard or have a qualified alternate delegating nurse available on call.
• Provide appropriate delegation of nursing tasks.
