Data Collection Coordinator

Responsibilities

  • Collaborate with Case Management, Physicians, Nursing, Patient Access, and Patient Relations to identify all patients who require an IM.
  • Issue (within 2 calendar days of the patients admission) an IM to the patient or the patients representative and obtain a signature if the patient has not received an IM in the Patient Access Department / Admitting. File a copy of the IM in the patients chart / record.
  • Call the patients representative (if not available) on the hospital telephone, and mail or fax the IM on the same day.
  • Collaborate with the Nurse Case Manager to identify patients who will be discharged. Re-issue the IM 48 hours prior to the patients discharge.
  • Notify the Case Management Department and the Nurse Case Manager of the unit should a patient request an appeal of their discharge.
  • Identify and participate in departmental P.I. activities, bringing any discrepancies to the attention of a supervisor.

Qualifications

  • A high school diploma or equivalent.
  • A minimum of one (1) year of healthcare or customer service experience.
  • Excellent communication & interpersonal skills.
  • Strong organizational skills.
  • Ability to work independently with minimal supervision.
  • Bilingual English / Spanish preferred.
  • Pay Range : USD $25.96 - USD $26.54 / Hr.
