Assurance Guardianship and Care Services is looking for a part-time Care Manager, with the possibility of becoming full-time and salaried. This employee will be an advocate for clients who have been deemed incompetent by the courts and / or elderly and disabled individuals who need support identifying and planning long-term care needs.
The care manager works closely with the team as they use their skill sets to work with an often-difficult population, including mentally ill, intellectually disabled, and aging individuals who have difficulties making independent decisions, while respecting their choices and maximizing independence in the least restrictive but safest way possible. A care manager must provide an assessment of needs before making decisions regarding the client’s medical consents, living arrangements, coordination of benefits.
The care manager is eligible to become a National Certified Guardian with the opportunity for advancement. This position requires candidates to conduct home / or facility visits for clients in and throughout the metro area. The position is primarily working remotely, though the candidate will be required to meet regularly with their supervisor. Initial training will be conducted on-site.
Duties :
- Care Managers assist in making decisions and following through on decisions made by clients such as living arrangements, medical decisions, quality of life improvements, etc.
- Manage and maintain a caseload including all required documentation in accordance with agency policy and procedure.
- Maintain confidentiality according to HIPAA.
- Make required visits to clients within their home setting (i.e., nursing home, group home, independent living, assisted living, apartment, house)
- Monitor and set up in-home client specific medications as needed.
- Attend interdisciplinary care conferences
- Complete assessments for clients referred to the program
- Participate in court and legal processes pertaining to clients as needed
- Telephonic On-Call Rotation with team 24 / 7 to provide consent to treatment for medical and other emergencies.
- Care Managers will be required to transport some clients to and from appointments, must maintain a clean driving record and insurance.
- Due to the remote nature of this job, it is critical that care managers possess time management and organizational skills.
- Having a cell phone is required.
Education / Skill Requirements :
Minimum bachelor’s degree in social work or another related field with three years experience. MSW, Nurse, or Master’s degree from an accredited institution in the related field working with diverse clients including elderly and those with mental health and intellectual disabilities diagnosis preferred.One-year social service experience working with individuals with mental health issues and / or developmental disabilities.Experience working with a variety of systems such as the courts, mental health, DD, medical, aging, protective services, and long-term care is preferred.Job Type : Part-time
