A company is looking for a Social Worker Care Manager.

Key Responsibilities

Support the mental and physical health care of patients on an assigned caseload

Screen and assess patients for mental health and substance abuse disorders, facilitating engagement and follow-up care

Document patient progress and treatment recommendations in the EHR and coordinate with medical providers

Required Qualifications

Minimum of MSW

Preferred Licensure or provisionally licensed

Behavioral Health experience and knowledge

Must be a licensed driver with an insured automobile in good working order

Time management skills are essential
