May be asked to help in other territories within facility's service area as needed which is about a 60 mile radius from the Facility address

RN productivity expectation for 40 hrs = 28 - 32 points / wk

SOC 2.5, ROC 2.0, Discharge (OASIS) 1.5, Recertification- 1.5, Routine visits- 1

Must have recent HH exp 2023-24

HomeCare HomeBase experience required

Client may consider without HCHB exp if the manager is able to provide training.

Any overtime must be pre-approved and requires written approval from Administrator
