Nurse Practitioner | Advanced Wound Care

Our nationwide team of skilled wound care specialists partners with skilled nursing homes, assisted living facilities, and long-term acute care hospitals to provide advanced treatments and reduce hospital readmissionsimproving lives and outcomes every day.

Were looking for passionate and proactive part-time (2-days a week) Nurse Practitioner to join our mission of healing and innovation.

What Youll Do

• Advanced Wound Care & Patient Assessment
• Conduct thorough assessments of chronic and acute wounds.
• Develop and implement individualized, evidence-based treatment plans.
• Apply advanced techniques such as debridement, dressing selection, skin grafts, and infection control.
• Collaborative Clinical Leadership
• Partner with facility healthcare teams to coordinate care plans and treatment updates.
• Ensure continuity of care through consistent communication and collaboration.
• Multi-Facility Travel & Scheduling
• Manage a travel schedule visiting multiple facilities weekly.
• Ensure timely and efficient care delivery with professionalism and compassion.
