About The Position

Job Overview :

Seeking an experienced Physical Therapist for a travel Home Health assignment covering Molalla, Mulino, Colton, Woodburn, OR, and surrounding areas. The PT will provide in-home therapy focused on functional improvement, mobility, and independence. This is a productivity-driven position with up to 6 visits per day (30 points / day).

Responsibilities :

• Perform physical therapy evaluations and develop individualized treatment plans
• Deliver therapeutic interventions including strength training, gait training, and pain management
• Educate patients and caregivers on safety and home exercise programs
• Complete documentation using Oasis and Epic (preferred)
• Travel up to 50+ miles as needed within the service area
