Refuah Health focuses on integrated, whole person care and serves a diverse patient population from children to adolescents, adults and seniors. We currently offer a wide array of behavioral health services, from individual therapy to couples and family sessions, with a focus on expanding our offerings to multiple psychopathological entities.
Essential Functions :
- Gathers and analyses information about individuals using interviews, case histories, observational techniques, and other assessment methods.
- Develops therapeutic and treatment plans, specifying type, frequency, intensity, and duration of therapy. Considers patient's interests, abilities, and needs in the development of the plan.
- Conducts regular therapy with patients.
- Communicates with patients' family members regarding ways they can participate and help with patients' treatment plan.
- Provides pertinent information about patients to providers, psychiatrists, nurses, and other disciplines who participate in the patients' treatment plan.
- Monitors treatment processes and makes changes when necessary.
- Refers patients to institutions, support services, and specialist as needed.
- Helps patient utilize the resources of their families and the community. This may be accomplished by either referring the patients to resources or acting as an intermediary on behalf of the patients in their dealing with other health and welfare agencies.
- Documents clinical and progress notes, summaries and other clinical record information.
- May confer with assigned supervisor on a regular basis to discuss measurable goals, patient management, and performance.
- Maintains confidentiality of information at all times.
- Complies with New York State Department of Health regulations as it relates to mandatory continuing education.
- Complies with applicable policies and procedures and supports the Compliance Program.
- Contributes to organizational operations by performing other duties as may be required.

What You Bring To Refuah :
Master's degree in Social Work from a Graduate School of Social Work accredited by the Council of Social Work EducationCurrent NYS License to practice Social WorkThree years of professional experience providing social casework. One year experience in a medical setting preferredSpanish speaking a plusExperience with trauma, substance abuse and pediatrics (preferred)CASAC certification a plusKnowledge of the principles, procedures, techniques, trends, and literature of master and clinical social workDisplays exceptional verbal and written communication skillsPossesses strong active listening skills and social perceptiveness abilitiesDemonstrates excellent negotiations, persuasion, and complex problem solving abilitiesPossesses the ability to make sound judgments and has good decision making skillsEligible to Work in the USApply Today :
