Join Our Team as a Licensed Clinical Social Worker (LCSW)

Are you ready to make a meaningful impact in the lives of patients and their families? At Carle Health, we are looking for a passionate Licensed Clinical Social Worker (LCSW) to join our primary care treatment team. In this role, you will provide essential psychosocial assessments, supportive counseling, crisis intervention, and education, all while collaborating with a dedicated healthcare team.

Your Responsibilities

As an LCSW, you will :

• Conduct comprehensive psychosocial assessments for patients and families, identifying emotional, social, and environmental strengths and challenges.
• Formulate and implement tailored care plans using effective social work interventions.
• Provide education and counseling to help patients and families adapt to their health situations.
• Utilize crisis intervention techniques and offer individual, group, and family therapies.
• Document findings and actions in accordance with departmental guidelines.
• Facilitate support groups and community-based activities.
• Assist primary healthcare providers in recognizing and treating mental health disorders.
• Research and develop resources to meet the social and emotional needs of patients.
• Connect patients with relevant community resources as needed.

Compensation and Benefits

The compensation range for this position is $29.29 - $50.38 per hour . This range reflects a good faith minimum and maximum for the role at the time of posting. Actual compensation will depend on various factors, including experience, qualifications, and location.
