📍 Location : VNA Hospice of the Southwest Region – Rutland, VT

📆 Contract Length : 12 Weeks

🕒 Shift : Monday–Friday, 8AM–4PM (Some weekends / on-call required)

💼 Guaranteed Hours : 37 / week

💵 Pay Rate : $70 / hr | OT : $80 / hr | Weekly : $2,590

🔍 Key Responsibilities :

• Conduct comprehensive patient / caregiver assessments
• Develop and implement personalized care plans
• Serve as Case Manager for hospice patients
• Participate in IDT meetings and interdisciplinary planning
• Deliver advanced symptom and pain management
• Delegate tasks per LPN scope and provide clinical supervision
• Educate patients and families on care plans, medications, and hospice services
• Coordinate with providers, facilities, vendors, and insurance for effective patient care
• Ensure timely documentation and compliance with State / Federal and agency regulations
