The Behavioral Health Provider serves as a member of the Integrated Care Team that provides comprehensive, integrated care at Baptist Community Health Services under the direction of the Director of Behavioral Health. The American Psychiatry Association describes integrated care as : a general term for any attempt to fully or partially blend behavioral health services with general and / or specialty medical services.
The Behavioral Health Provider for children, adolescents, and adults (ages 6 years old and older) provides one-on-one therapy encounters on a daily schedule and collaborates in planning and supporting initiatives that foster the integration of mental health services into the holistic primary care promised by the Medical Home Model. This includes supporting the culture of collaboration among professional disciplines, coordination of care and problem solving with cultural sensitivity to patients beliefs and capacities. Champions a team attitude and contributes in meetings related to the BH Department and the rest of the BCHS team.
Completes thorough biopsychosocial assessments to assess patients mental health status; and patients and familys emotional, social, and environmental strengths; and treatment plans with each patient
Develops plans of care based on psychosocial assessment findings
Uses brief one-on-one interventions or group sessions to assist patients in managing mental health issues
Assists with crises, while providing appropriate interventions
Arranges, accesses, and coordinates patient / family pre and post crisis care
Prepares and supports patients and families in dealing with stressful circumstances and crises in their lives
In electronic health record, complete clear notes including treatment goals / plans. Closes charts same day (maximum time is 48 hours to close charts)
Completes documentation of patient care that supports prompt revenue cycles at BCHS
Sees patients in clinics, schools, or other community locations
Works closely with primary care providers and other clinical staff to improve coordination in the treatment and management of patients,
Maintains working knowledge of and liaison with community agencies and resources
Maintains a working knowledge of relevant medical / legal issues that impact patient care, e.g. advance directives, child and elder abuse, mandatory reporting
Participates in regular supervisory conferences; keeps current with social work and health care developments and enhances professional practice and knowledge
Provides clinical training and onboarding training to new behavioral staff, as needed
Supervises students, if appropriately licensed.
Participates in peer reviews and credentialing and privileging process
Upholds state licensing board requirements
Counsels patients / families, who are diagnosed with HIV or are at-risk for HIV
Counsels patients living with HIV around issues related to adaptation to diagnoses, illness, treatment and / or life situation
Performs other duties as assigned
Work is normally performed in a clinic, office environment, school, mobile unit, and / or telehealth.
