Position Type : Full Time, 12 hour shifts

Available Shift : 6 : 30PM-6 : 30AM with rotation weekends

Responsibilities :

• Provide direct patient care, including administering medications, performing treatments, and documenting patient information
• Collaborate with other healthcare professionals to develop and implement individualized care plans
• Monitor and report patient conditions, concerns, and changes to the interdisciplinary team
• Participate in resident assessments, care planning, and discharge planning
• Maintain accurate and timely records of patient care and communication
• Demonstrate a commitment to delivering excellent customer service and promoting a positive patient experience
