About The Position

Must-Have : Proficiency in OASIS and HomeCare HomeBase EMR, must have been used within the last 6 months

Description :

Provides coordinates and directs the provision of home nursing care according to physicians' orders.

Based on agency policies and procedures through the competent application of the nursing process.

Responsible to Clinical Manager.

Directly supervises and evaluates Home Health Aides and Licensed Practical Nurses / Licensed Vocational Nurses.

Home visits required.

Periodic office meetings required.

Must have reliable transportation and a current driver's license.

Agency-required liability insurance is necessary.

Average number of patients per day : 5-8.

The contractor will be assigned between 5-10 patients per day.

Patient visits equate to roughly 6-8 points.

Types of cases include Start of Care, Wound Care, Follow-up Visits, and more.

Patient documentation must be completed within 24 hours of the appointment.

Candidates must be open to on-call rotations.

Candidates should be willing to travel up to 75 miles from the home clinic.

Local candidates are accepted at the same rate.

No contract hours are guaranteed.

Weekly productivity expectations include 30 points.

Delayed and incomplete documentation that impacts billing and follow-up patient care will result in cancellation.

Please confirm that the clinician understands the expectations of the role.

This includes accepting all patients on their schedule and the travel requirements.
