Job Description

Seeking BE / BC Family Medicine or Internal Medicine Physician with a passion to care for the elderly / senior population, or Geriatrician to provide primary care services to SR Care clients at participating LTC facilities and serve as the individual SNF's Medical Director.

• Work / Life fit : Enjoy a flexible, full time 4 or 5 day workweek with admin time, no after-hours call, no weekends
• Work Environment : Integration and management of care to include inpatient admitting, clinical, and admin services.
• SNFs Leadership : Shine as the Medical Director to our contracted SNFs.
• Advocacy : For patients and providers. Administration round tables with providers for input with strategic planning
• Teaching-Friendly Culture : Share your passion with residents and medical students
• Expected Volume : Seeing 14 patients assigned per day, typically at 1 facility per day.
• Longevity and Reputation : group has been a trusted partner in the health and wellbeing of our communities for over 30 years!

• Collaborative Support : From the SR Care Program Director, residents, physicians, ARNPs, medical assistant, and program assistant
