Supplemental Health Care is hiring experienced Home Health LPNs (1+ year in a Medicare-certified agency) for contracts in Chesapeake, Virginia, and nearby areas.

Key Responsibilities :

Provide skilled nursing services under the direction and supervision of an RN.Monitor patient status and immediately report changes to the supervising RN or physician.Accurately document all care provided in compliance with agency policies and state nurse practice acts.
