Job Summary and Responsibilities

As a Registered Nurse with American-Mercy Home Care youll be at the heart of patient care delivering expert clinical services coordinating with multidisciplinary teams and building meaningful relationships with those you serve. Responsibilities :

• Deliver compassionate high-quality nursing care tailored to each patients needs.
• Develop and adjust individualized care plans to ensure optimal patient outcomes.
• Collaborate with physicians therapists and other healthcare professionals.
• Supervise your own caseload nurturing the growth and development of LPNs LVNs and Home Health Aides to facilitate and cultivate a team of caregivers.
• Provide education and support to patients and their families.
• Maintain thorough documentation to meet regulatory and operational standards.
• Utilize the latest technology and best practices to enhance patient care.
