Licensed Practical Nurse | AMs, PMs, NOCs (overnight) | Part-Time
This job has pay differentials for PMs and NOCs. PMs $1.00 and NOCs $1.50

- 64 hours biweekly, 6am-2 : 30pm, rehab, every other weekend and every other holiday required
- 40 hours biweekly, 6a-2 : 30pm, memory care, every other weekend and every other holiday required
- 16 hours biweekly, 6a-2 : 30pm, memory care, every other weekend and every other holiday required
PMs

Understands resident diagnosis and helps to establish appropriate approaches and goalsAssists with the development and revision of individual resident care plansInterprets resident care plans to other LPNs, Nurse Techs, Medication Technicians and CNAs.Communicates resident status through documentation and verbal reports. Reviews charts, orders and nursing care plans on a regular basis.Provides direct basic resident careParticipates in care conferences in order to provide input into multi-disciplinary resident plan of care.Collects data for resident assessments.Administers medications and treatments according to physician orders and standards of practice.Assists physicians during treatments and during “physician progress visits”.Performs complex nursing functions under the supervision of an RN once competency has been verified.Collects data for all residents when change of condition is noted and reports to the RN for follow up assessment.Prepares work schedules and work assignments for other LPNs, Nurse Techs, Medication Technicians and CNAs.Completes competencies of Nurse Techs, Medication Technicians and CNAs and demonstrates nursing techniques and procedures under the supervision of an RN.Monitors use and safety of equipment and supplies.Instructs residents and family members when necessary.Informs Clinical Coordinator / Director of Nursing and Nursing Home Administrator of resident changes or facility emergencies as needed.Participates in QA / QI process.Communicates and cooperates with other departments to ensure a continuum of quality care.
