Unit Description : Palliative Care is seeking an experienced RN to work with our team. This role works as part of the multidisciplinary team, providing care to patients with serious illness. More specifically, this role will triage new consults, assist in the assessment of patients, lead goals of care conversations with patients and their families and communicate and collaborate with team members regarding patients who are clinically complex and / or on a comfort directed plan of care. In addition, this role will serve as an educational resource to inpatient nursing staff in the caring for patient with serious illness and their families.
Experience : Greater than one year of experience.
On-call : Not required.
