Overview

The Hospice Clinical Supervisor plans and coordinates program development under the direction of the Director . Serves as an experienced clinician in the provision of care to Hospice patients of all ages . Provides supporting supervision for day-to-day operations of clinical team s including clinical leadership for staff as assigned . Leads interdisciplinary programs / teams and coordinates new program development under direction of the Director . Collaborates with Hospice Director Medical Director and physicians to ensure patient care and safety expectations are met . Promotes staff development and education . Embodies the Memorial Health Values of Safety Integrity Quality and Stewardship that support our mission and vision.

Qualifications

Education :

  • Registered Nurse

Licensure / Certification / Registry :

  • Current license to practice professional nursing in the state of IL.
  • Certification in a hospice / palliative care field preferred.
  • Current CPR certification required.
  • Must possess a valid IL drivers license and must be deemed as an acceptable driver in accordance with the MHS fleet safety policy.

  • Responsibilities

  • Ensure fulfillment of mission and progress towards vision. Exhibit behaviors consistent with MH values and ensure strategic plan is cascaded throughout the organization to achieve desired business outcomes.
  • Assist in maintaining a cohesive leadership team. Provides l direction and support for assigned members of the team including onboarding of new colleagues monthly updates triaging of work appropriate staffing levels expectation setting resource allocation and follow through of assignments. Interprets polices objectives and operational procedures and represents them in a positive manner.
  • Coaches and mentors colleagues to their fullest potential including assistance as able with educational and developmental opportunities. Leads colleagues to fully utilize the time talent technology and resources available to them to ensure maximum effectiveness and efficiency. May annually evaluate the performance of any assigned colleagues. Assist Hospice Director in any development and implementation of succession plans for the team.
  • Actively participates as a contributing member of the Interdisciplinary Team. Assists in Development and enhancement of strategic partnerships with various internal and external stakeholders including but not limited to universities schools systems government entities and key business partners.
  • Assists and supports the for creation of an organizational hospice infrastructure necessary to build staff knowledge skills and behaviors to help meet current and future demands and support the strategies and business goals of the health system.
  • Support strategic direction of the organizations aspiration to be the health system people choose over all others . Seek out and drive best practices throughout the Health System. Monitor quality and effectiveness of these processes.
  • Build trust and foster collaboration and communication with the clinical staff. Meets regularly with staff to encourage open communication request input and provide feedback.
  • Aides in ensuring compliance with all regulatory agencies governing health care delivery and the rules of accrediting bodies by continually monitoring all operations programs and physical properties; initiates changes when required.
  • Provides guidance to ensure that Hospice provides the highest quality most cost-effective healthcare possible and that operations are in compliance with the Joint Commission guidelines as well as federal state and local laws and regulations.
  • Strives to continue professional growth and development through continuing education seminars workshops and professional affiliations.
  • Perform other related work as required or requested.
