The PT will evaluate, plan, and deliver evidence-based interventions to help patients regain function, improve mobility, and achieve independence. This role requires excellent clinical judgment, documentation accuracy, and collaboration with a multidisciplinary care team. Responsibilities Evaluate and develop individualized treatment plans for patients with physical impairments, mobility challenges, or wound care needs per regulatory and clinical practice standards. Apply evidence-based techniques and standardized assessments to guide therapy interventions. Administer treatments such as manual therapy, therapeutic exercise, ambulation training, modalities, and use of assistive devices. Monitor and document patient progress, adjusting plans as needed to achieve clinical goals. Provide education and counseling to patients, families, and caregivers to promote carryover of care. Communicate effectively with supervisors, nursing staff, and interdisciplinary team members regarding patient progress and treatment plans. Complete accurate and timely documentation within the electronic medical record system per regulatory standards. Participate in required care conferences, utilization reviews, and facility meetings. Supervise and support PTAs, rehab aides, and students as applicable. Adhere to APTA standards , state practice acts, and professional codes of ethics.
