HOSPICE CHAPLAIN

(Harnett, Johnston, Wayne)

Job Description :

• Provides spiritual support to patients / caregivers.
• Develops and maintains a resource group of clergy.
• Acts as a liaison between hospice and community.
• Documents records of visits to patients / caregivers.
• Works collaboratively as a member of the Interdisciplinary Team.
• Other job duties as assigned.
