ECM Nurse Care Manager

A Greater Hope, the Inland Empires leader in community-based services for youth and families, is seeking a value-aligned Registered Nurse to support our Enhanced Care Management (ECM) program. This innovative service model is transforming the way communities care for our most vulnerable populations, and we need a compassionate innovator to support this new model.

The Registered Nurse Care Manager will work with a multi-disciplinary team, holding a direct caseload as well as consulting with colleagues on the health care needs of all ECM clients. This role involves assessment, case planning and coordination, linking clients to all necessary services for whole-person care. Much of this role will be in-person, in the community.

Starting Salary Range :  $105,000-$120,000

Qualified candidates should be highly organized and have at least three years of clinical nursing experience, including case management in a managed care or healthcare setting.

Requires a Bachelors Degree in Nursing (BSN), with a Masters Degree in Nursing (MSN) or a related field preferred.

Must have an active and unrestricted RN license in the State of California,.

Bilingual skills are highly desirable.

Compensation details : 105000-120000 Yearly Salary
