A company is looking for a Newborn / Pediatric Nurse Care Manager who is passionate about holistic member care and guiding them through their healthcare journey.

Key Responsibilities :

Deliver coordinated, patient-centered virtual care management via telephone and video

Create impactful care plans with members and the care team to achieve health goals

Assist members in navigating complex medical conditions and the healthcare system

Required Qualifications :

Bachelor of Science in Nursing (BSN)

Current CCM Certification or eligibility within 6 months of hire

5+ years of nursing experience

2+ years of experience in care management

Active Compact RN license in good standing
